Client Bill of Rights

The Client Bill of Rights will be posted on the wall at the office for you to read at your leisure.

Co-parenting/Custody

Please note that in situations where parents have joint legal custody, both parents have the right to the child’s information and the therapist will share the treatment plan with both parents.

If you have full legal and physical custody of the child you will need to be prepared to provide documentation of this.

If you are a legal guardian please bring copies of documents granting guardianship to the first appointment.

Initial Assessment $220

Payment

Options for payment include:

  • Billing your insurance provider
  • Private Pay
  • We accept cash, checks, and credit card payments
  • If you have a copay, it is due at the time of service. If you choose to use your insurance, you are responsible for verifying your coverage. We can bill most insurance providers but that does not guarantee payment, and you are responsible for payment on your account. Insurance requires a medical diagnosis for coverage. After the initial session, if you do not meet criteria for a medical diagnosis, you would have to pay out of pocket for any continued services. Also be aware, that your insurance plan may exclude payment for certain diagnoses and you would be responsible for costs.
Private Pay

Clients not using an insurance provider must pay in full at the time of service, unless a payment plan has been previously arranged.

Past Due Accounts

An account is considered past due after the 60 day grace period. Accounts with a balance of $400 or more may be terminated, or be discussed with your therapist. If you are unable to pay the full amount, please discuss a payment plan with your therapist.

Cancellations/No Shows

A missed appointment is a missed opportunity for you and for someone else who could have used that appointment time. It is also a misuse of your therapist’s time. Appointments must be canceled 24 hours in advance of your appointment. Late cancels and no-shows will be charged $60, except in the case of emergencies or illness. In case of illness, please notify us no later than 9:00a.m. the day of the appointment. Please leave a message if you get our voicemail.  Insurance companies will not pay for no-show or late cancellation fees.

Crisis/Emergencies

If you, or someone you know, is experiencing a crisis (an unstable situation) during business hours please call the office at 320-774-1621. If your therapist is available you will be directed to that therapist. If not, you will be asked if you would like to speak to an available therapist. If it is after office hours you may call the St. Cloud Hospital at 320-251-2700 or the Mental Health Crisis Hotline at 320-253-5555 or 1-800-635-8008. If it is an emergency situation that requires immediate action, call 911 or go to the nearest emergency room.
