Our office is located in the Midtown Square Mall at 3333 W. Division St. Suite 119. Parking is located in the back of the building. Upon entering the building by Sta-Fit, our office is located to the left of the stairs and elevator next to Assured Health. We are located on the bus route and the building is handicapped accessible.

The receptionist will ask for your contact information and what type of insurance you have, if you would like to use your insurance.
